There are TWO necessary documents required to complete the application process.

1. The Elan Application which is an online form represented below

2. The Elan Health and Permissions Release Form which is attached below and must be completed and attached in the necessary field of the online form

BIOGRAPHICAL INFORMATION
Please write a short 1 page bio including experience, achievements, education, current and former teachers, and repertoire (solo, orchestral and chamber music) and attach the document at the bottom of this form.

NON-REFUNDABLE APPLICATION FEE OF $50
Application deadline May 30, 2012.

Upon notification of acceptance, a $350 deposit must be submitted with the balance due by June 5, 2012.

No refunds will be made after June 10, 2012.
